Each GP practice has a defined area within which they will accept new patients. 

Before filling out a registration form, check you are in our catchment area.

Registering with the surgery

To register with the surgery:

  • Fill out an online registration form
  • Follow this link to print a paper registration form - please complete this and bring it into your practice
  • Visit reception to collect a paper registration form and return it 

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Please make sure that all the personal details are filled in clearly, including the details of your previous GP surgery, as this makes it much easier to identify you on the national system.
